It has come to my attention that you were involved in an incident and your actions may be in violation of the Code of Student Responsibility, specifically:
- Disorderly, Violent, Intimidating or Dangerous Behavior to Self or Others
- Judicial Process Compliance

A student by voluntarily joining the University community agrees to abide by the standards that have been instituted by DePaul University. Students are responsible for being familiar with all of the policies and procedures included in the Code of Student Responsibility.
It is critical to your continued success at DePaul University that we meet to discuss this situation. You can contact the Dean of Students Office front desk at 773-325-7290 in order to schedule a mandatory meeting that may begin the Judicial Review Process. This meeting must occur no later than February 15, 2013.
You are prohibited from having any contact with the 13 individuals named in the Public Safety Supplemental Investigation Report (Case #N13-01-52) that you were given by Domonic Rollins, Assistant Dean of Students. This contact includes, but is not limited to: in person, email, text message, instant message, the internet, phone, through other people or on DePaul owned or affiliated properties.
Additionally, in preparation for this meeting, it will be vital that you are familiar with the following aspects of the Code of Student Responsibility:

- Judicial Sanctions
- General Information about the Judicial Review Process
- Explanation of Important Aspects of the Judicial Review Process
- Please pay special attention to the "Advisor" portion for this website. If you choose, you have the right to bring an advisor to your meeting.
